NAFLD Screening for DM/HTN Patients
All diabetes and hypertension follow-ups must now include NAFLD screening using FIB-4 score or abdominal USG. Documentation via the new template in CCMS.
What's changing
Starting 15 May 2026, every diabetes (DM) and hypertension (HTN) follow-up consultation must include screening for Non-Alcoholic Fatty Liver Disease (NAFLD). This applies to all NCD Unit follow-up sessions at KK Bandar Maharani and other clinics under PKD Muar.
Why
Recent local audit data (PKD Muar, Q1 2026) showed that an estimated 38% of DM/HTN follow-ups had undiagnosed NAFLD — a significant predictor of cardiovascular morbidity that's currently being missed in routine NCD care. Aligning with the 2025 MOH NAFLD Clinical Pathway, screening at every NCD review captures these cases earlier.
Action items
- Calculate FIB-4 score at every DM/HTN follow-up using the new template in CCMS (Module: NCD → Templates → "NAFLD Screening — FIB-4")
- If FIB-4 ≥ 1.3: refer for abdominal USG within 4 weeks
- Document the result in the CCMS NCD episode (don't use free-text notes — use the structured field so the audit query can pick it up)
- Counsel patients with risk factors on lifestyle modification regardless of score
