Area covered: Registration, Vitals & Appointment Counter, Outpatient Department, Fever Clinic etc.

Trigger

CCMS/Systmone is inaccessible at service points. No internet or other causes that leads to CCMS unusable --> BCP Manager announces Downtime Activation.

Register Patient

Manual or backup electronic registration

Registration

Register patient details in Manual Registration Book or EPRS (Electronic Patient Registration System — legacy backup system).

Issue Manual Clerking Sheet

Patient basic details pre-filled

Issue Manual Clerking Sheet with patient basic details written on it.

Skip Checklist

Standard unit assignment checklist not required during downtime

No need to issue Checklist — the physical checklist normally used for unit assignment is skipped during CCMS downtime.

Determine queue system availability

Continue using QMS calling system as normal. Staple QMS number onto the Manual Clerking Sheet for tracking.

Assessment & Documentation

Conditional Steps

Depending on the clinical plan, one or more paths apply:

If Procedure Required

  • Update procedure on clerking
  • Direct patient to Procedure Room
  • Lab: write type of test on clerking

If X-Ray Required

  • Fill-up Borang Permohonan X-ray
  • Update clerking sheet
  • Refer patient to Room 7

If Referral Needed

  • Prepare Emergency Referral Form (2 copies)
  • Update clerking sheet
  • Refer to Emergency Room 8

Appointment Counter

  • Assign appointment date based
  • Record appt in manual logbook
  • Provide TCA card to patient
  • Collect Manual Clerking Sheet

Post-Test/Procedure Review

After patient returns from investigation

  • Review outcome
  • Complete assessment
  • Diagnosis & treatment plan
  • Set appointment date (TCA)
  • Write manual prescription

Record Collection

Mandatory

All BCP records (clerking sheets, prescriptions, consent forms, logbooks) are collected by Unit BCP Coordinators.

Data re-entry begins

DATA RE-ENTRY

Once CCMS is restored, data re-entry begins following unit-specific procedures.
