CCMS Downtime Confirmed — SystmOne is inaccessible --> BCP Manager announces Downtime Activation.

Patient Entry to Procedure Room

Patient arrives via 2 possible routes:

Direct Visit for Procedure

Route A

Patient arrives with a lab form and a TCA (To Come Again) appointment date for a scheduled procedure.

Referral from Doctor's Room

Route B

Doctor refers patient for procedure during consultation. Doctor updates the test/procedure required on the patient's Manual Clerking Sheet.

Perform Procedure

Clinical procedure carried out as ordered

Staff performs the procedure on the patient: blood taking, injection, medication administration etc.

Document on Manual Clerking Sheet

All details and outcomes recorded on paper

Details and outcome are recorded on the patient's Manual Clerking Sheet.

Log Workload into Excel/Google Sheet

Parallel digital tracking for recovery

Google Sheet

Staff logs workload into Excel/Google Sheet with:

• Patient Name
• IC Number
• Procedure Type
• Outcome

Completed Test & Procedure

Procedure Complete

Procedure is completed and documented. Patient now proceeds to the appropriate next step based on clinical need.

Post-Procedure Pathways

Patient will be further divided into these 2 pathways

Returns to Doctor's Room

Path A

• Action: Doctor reviews the procedure result and proceeds with the next clinical action.
• After Completion: Collect manual clerking sheet for record retention and data re-entry.

If Patient Needs Referral

Path B

• Action:Doctor provides written Emergency Referral Letter (2 copies).
• Escort Preparation: Prepare escort and ambulance for patient transfer to hospital.

Recovery: Once System Restored

CCMS comes back online

ICT in-charge confirms system restoration with BCP Manager.

Data Re-Entry Begins

All manual records entered into SystmOne

IMPORTANT

Procedure Room staff begin back-dated data re-entry into CCMS using the Manual Clerking Sheets and Excel/Google Sheet logs.
