Wound Care Clinic Hub

The Wound Care Clinic (WCC) at Klinik Kesihatan Bandar Maharani delivers structured, protocol-based wound care management through the T.I.M.E. assessment framework and evidence-based dressing selection. The service operates from a dedicated facility (Room 9) under clinical supervision with full SystmOne documentation integration.

General Patient Journey

Triage & Registration

Patient arrives at Triage Counter A and is directed to the OPD Registration Counter. After registration, the patient is allocated to Wound Care Clinic Room 9.

Vital Signs & Assessment

Vital signs and anthropometry are recorded using the JKN Assessment & Procedure Template. The patient then waits in the Room 9 waiting area.

Wound Assessment

The wound care team calls the patient via SystmOne and performs a T.I.M.E. framework evaluation. Complex or deteriorating wounds are discussed with the MO in charge.

Dressing Procedure

An advanced dressing is selected and applied based on wound characteristics. Serial images are captured for progression tracking, and consent is documented.

Documentation & Follow-up

The Rekod Penjagaan Luka template is completed in SystmOne. The next appointment is scheduled, and the patient is discharged with wound care instructions.
