How to Fix a Wrong IC Number

What Problem Does This Solve?

A patient was registered with an incorrect IC number — whether due to a typo during registration, a child mistakenly registered under the mother's IC, or an ID change after document renewal. This procedure corrects the IC and merges any resulting duplicates.

Before You Start

Prerequisites
  • Administrator or Front Desk Supervisor role
  • The correct IC number for the patient
  • Verification that the new IC belongs to the same person (same DOB, name, photo)

Step-by-Step Instructions

Open the incorrect patient record

Search and retrieve the record with the wrong IC

  1. Use Quick Patient Search (Ctrl + F).
  2. Search by the incorrect IC or the patient's name.
  3. Open the record that needs correction.

Navigate to Change IC Number

Access the security-controlled procedure

From the patient record menu, go to:

Patient → Patient Maintenance → Security Controlled Procedures → Change IC Number

Restricted Access

This menu is only available to administrators and front-desk supervisors.

Enter the correct IC number

Type the accurate identification number

  1. In the Change IC Number dialog, enter the correct IC.
  2. Click Next to proceed.
Double-Check

Verify the IC number twice before proceeding. Once changed, the system may auto-merge if a duplicate exists.

Review the merge dialog

System shows both records if a duplicate exists

If the correct IC already exists in the system, the merge dialog appears showing:

  • Record A (current record with wrong IC)
  • Record B (existing record with correct IC)

Confirm both records belong to the same patient before merging.

Cannot Be Reversed

Merging cannot be easily undone. If you are uncertain, cancel and verify with the patient first.

Confirm and complete

Finalize the IC change and merge

  1. Click Confirm Merge to consolidate records.
  2. All data from both records is combined into the correct IC record.
  3. A success message confirms completion.
Result
  • Patient now has the correct IC number
  • Duplicate records merged into one
  • All historical data preserved

Troubleshooting

Problem: A newborn was registered using the mother's IC number instead of leaving the IC field blank.

Solution:

  1. Open the child's record.
  2. Use Change IC Number to enter the child's actual IC (once issued).
  3. Confirm the merge if a duplicate exists.

Problem: Registration clerk made a typo (e.g., 900101-01-5555 instead of 900101-10-5555).

Solution:

  1. Open the record with the typo.
  2. Use Change IC Number to correct the digits.
  3. If the correct IC already exists, review and confirm the merge.

Problem: The same patient was registered twice — once with an old ID and once with a new one.

Solution:

  1. Open the record with the outdated IC.
  2. Change the IC to match the newer/correct one.
  3. Confirm the merge dialog to combine both records.
